Please find below, the information containing links for the PA State Mandated Background Clearances.
All volunteers that will be coaching or assisting are required to complete/submit these clearances.
Remember each person needs 3 documents:
- PA Criminal Clearance PA State Police Criminal History
- PA Child Abuse Clearance PA Child Abuse History Certification
- FBI Fingerprint (if they have not resided in PA for the past 10 consecutive years) FBI Criminal History Background Check
-OR-
complete and sign a copy of the Volunteer Disclosure ( copy attached)
Please remember to save and print copies of each completed form.
Copies must be given to Head Coaches which will submit a team package.
